Program Development Form
This is the plan of your graduate course of studies. It also serves as your petition if you wish to transfer credits from another institution or program. Changes to your plan of study should be made in consultation with your advisor. Submit to Kelti Baker with your personal statement at time of application.

Application for Admission to Candidacy Form (Graduate Degrees Only)

This is a contract that must be submitted when you complete 18 credits in either Masters degree. It is very similar to the Program Development Form, but you cannot change your schedule after this form has been submitted without first discussing the change with your advisor and submitting a Request for Academic Adjustment form. Transfer courses desired to be used in your degree program must appear on the Program Development form and be approved at that time.

  • Fill out and mail the original form to Paul Castelin. Please allow us several days to obtain required signatures for you.
  • The Candidacy form must be in the graduate office by the deadline for the semester you plan to graduate or you will not be able to apply online for graduation.  BroncoWeb is set up to block applications for graduation unless the Candidacy form has been recorded as received by Graduate Admission and Degree Services.
  • Candidacy forms must be filled out completely.  This means that all credits needed must be listed on the form, even if they have not yet been completed, including Thesis or Project credits. Your culminating activity of either Portfolio or Comp Exam should also be listed, even though the comp exam is non-credit.

The Application for Admission to Candidacy form also shows your catalog year--this is the year you entered the program, or a later year (if you so choose). If you do change your catalog year after you have once submitted your AAC, you will need to file a revised AAC and meet all the requirements of the catalog for the year you selected.

Proposed Plan of Study for a Graduate Certificate Form (PPSGC) (Certificates Only)

This is a contract that must be submitted immediately after you have enrolled in your certificate program.  It is very similar to the Program Development Form and the Application for Admission to Candidacy (AAC) form, but if you need to change your schedule after this form has been submitted, you must first discuss the change(s) with your advisor and then submit a Request for Academic Adjustment form. Transfer courses desired to be used in your certificate program must appear on the Program Development form and be approved at that time.

  • Fill out and mail the original form to Paul Castelin. Please allow us several days to obtain required signatures for you.
  • The PPSGC form must be in the graduate office by the deadline for the semester you plan to graduate or you will not be able to apply online for graduation.  BroncoWeb is set up to block applications for graduation unless the PPSGC form has been recorded as received by Graduate Admission and Degree Services.
  • The PPSGC form must be filled out completely and accurately.  This means that all credits needed must be listed on the form, even if they have not been completed.  However, DO NOT just list your degree courses and expect the Graduate College to figure out which courses apply to your certificate. List ONLY the courses required or elected for your certificate.

Application for Graduate Independent Study

Graduate Independent Study is often used when a student wishes to pursue advanced study in a relevant area of interest. The student works with a high degree of independence to meet well-defined goals under the supervision of a member of the graduate faculty. A sample form based on an actual project is provided. A project proposal, detailed plan, and specific list of deliverables need to be worked out with the supervising faculty member prior to submitting the application.

Application for Written Comprehensive Exam (Submitted online only)
Submitted when students are nearing completion of 27 credits.

Graduation Application (Submitted online only)

After the AAC form has been processed you will need to Apply for Graduation in BroncoWeb. To do this,

  1. Login to BroncoWeb, click on Student Center, then, under the Academics banner, click on the drop-down box containing the entry, “other academics…”, then select Apply for Graduation.
  2. The system will do a check to be sure you have the AAC form on file. If you do not, you will not be able to Apply for Graduation. Call the Registrar Help Desk at (208) 426-4249, or regmail@boisestate.edu to determine your status and what needs to be done.
  3. During the application process you will also be asked to pay a $25.00 diploma fee for a degree, $20 for a certificate. You may use your credit card to pay.
  4. Separate applications are required for a degree and a certificate. Please check the academic calendar on the Registrar's website to be sure that you meet the deadline for applying: http://registrar.boisestate.edu/calendars.htm.

Use this form to request an adjustment of degree requirements (i.e., you change degree programs, or you change planned course work after the Candidacy form has been submitted).
