This is a map or a plan of your graduate career based on the catalog year you entered your program. It also serves as your petition if you wish to transfer credits from another institution or program. Changes to your plan of study should be made in consultation with your advisor.

This form should be signed and submitted to Jerry Foster with your personal statement at time of application. He will assist you in completing the form, if needed.

Changing catalog years: Students always have the option to change to a catalog year that comes after the year they entered the program. To change catalog years, you must file a revised Application for Admission to Candidacy. It is not possible to change to an earlier catalog year.

Choose the appropriate form based on your degree/certificate and catalog year.
